First Visit – What to Expect
Typically your initial appointment will consist of a comprehensive oral examination including an oral cancer screening, a prophylaxis (cleaning), and x-rays. Treatment recommendations will be made at this time and we can answer any questions or concerns you may have. If there has been an extended period of time since your last cleaning an additional appointment may be necessary to complete the prophylaxis.
You can assist us by providing the following information at the time of your appointment:
- New Patient Paperwork
- X-rays (if available). (Please see below.) These must be requested from your former dentist by you. There is a request form available here.
- A list of current medications that you may be taking including dosage and frequency (Please alert the office if you have a medical condition or any special needs that may be of concern prior to treatment. While our office is handicap accessible, there are medical and physical conditions that may require special scheduling.)
- Dental insurance information.
IMPORTANT: A parent or guardian must accompany all patients under the age of 18 at every visit. A treatment consent form is available here if you are unable to do so. It is valid for one year from the date it is signed.
XRAYS: Insurance companies have limits on how often they will pay for x-rays. If you have recent x-rays from another office (within one year), you may request that they be forwarded to our office. Health privacy legislation (HIPAA) requires that you make the request for the transfer of any medical records. The dentist may require a signed release and a duplication fee may be charged. If there is not enough time, please pick them up and bring them with you on the day of your visit. A record request form is available here.
